Proactive, data based insights, to help members improve or fine-tune their health in an effort to prevent or reverse chronic disease

If other medical specialists are needed, we will make those referrals and help navigate those interactions

$100 One-time registration fee for all new primary care patients

PLAN BENEFITS
This plan includes:

Comprehensive annual physical to provide a deeper insight into your current health status and ideas to prevent future health issues, PLUS

Two (2) additional visits are included in the cost of the plan.
Our traditional primary care services can be accessed in the 2 additional visits that are included in the plan. They are there when you need them, whether for a sick visit or a visit to fine tune your health. Visits must be used within 12 months. If you need more than 2 additional visits, add-on visits can be purchased.
Direct communication with your doctor and a personal relationship with your health care team.
If other specialists are needed, we will make those referrals and review recommendations and next steps if needed.
STAY WELL PLAN FEE
$1200 per year
$100 One-time registration fee for all new primary care patients
Add-on visits are $200 per 30 minutes or $350 per 60 minutes

ROOT CAUSE SOLUTION FEES

Option 1

$450 for the initial appointment (usually 60 minutes)

$200 for follow up appointments (typically 30 to 45 minutes; most patients need 3 to 4 visits)
Option 2

$950 total that includes the initial appointment and 3 follow-up visits
If you want to become a member of the Primary Care practice while engaged in this service, a portion of your Root Cause Solution fee can be applied to the Bridge Primary Care Membership. This can be discussed with your provider at the time of your appointment.
